If you are living in the practice area please contact reception to pick up registration forms and make an appointment with a healthcare assistant for a new patient health check.
When you attend this appointment please bring a copy of any repeat medication you may be taking.
Once you are registered you will be sent a medical card which will have the name of the doctor you have been registered with, however you have the right to express a preference and we will endeavour to accommodate you. (Subject to availability)
Registered patients aged 16 to 78 have a right to an appointment regardless of whether you have been seen or not over the last 3 years. You may be asked to attend a patient health check.
Registered patients aged 75 years and over have a right to an appointment regardless of whether you have been seen or not over the last 1 year. You may be asked to attend for a patient health check.
